RMA Return Procedure

Please make sure that all of the steps noted below are accomplished before your removed components are returned to Capstone Turbine Corporation. Always use the Capstone Return Material Authorization (RMA) forms. This will ensure that your returned material is properly received at Capstone Turbine Corporation.

! Fill out a copy of the Field Service Report (512298) completely. Describe the system and/or the component history and failure.

! Obtain a Return Material Authorization (RMA) for removed material from Capstone Turbine Corporation.

! Properly crate all materials for return to Capstone. Use Capstone issued crates when provided.

! Clearly, mark the RMA number on the outside of crate and/or package. DO NOT mark directly on

the components.

! Attach a completed copy of the Return Material Authorization form and the Field Service Report

(512298) with the returned package.

! USA and Canada: Return materials within 30 days from repair date. Arrange return shipments

through NuBorn Express. You may contact NuBorn Express at:

! Europe: Return materials within 30 days from repair date. Arrange return shipments through Hitachi Transport System at:

! Japan: Arrange return shipments through your preferred carrier or Hitachi Transport System.

• When using your preferred carrier, the RMA must be received at Capstone (USA) within

30 days of repair date.

• When using Hitachi Transport as RMA carrier, the RMA must be received at Hitachi Transport (Yokohama, Japan) within 30 days of repair date. You may contact Hitachi Transport System at:
